Notice Regarding Availability of Form 1095-C

In accordance with IRS requirements, Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, is available to eligible employees. Form 1095-C provides information about the health coverage offered to you by St. Charles Community College during the prior calendar year. This form is not required to be filed with your federal income tax return but should be retained with your tax records.

How to Access Your Form

Your Form 1095-C is available electronically through the Benefitfirst portal at: www.benefitfirst.com
Once logged into Benefitfirst, Form 1095-C is available under the Document History
section of your Benefit Statement.

Requesting a Paper Copy

You may request a paper copy of your Form 1095-C at no cost by contacting Human
