Accommodations Request for Returning Students

Returning Students now have two options for requesting accommodations:

• Submit/print online request
                   or           
• Make an appointment with accessibilities coordinator

Returning students are encouraged to process the Accommodations Request Form as soon as possible upon registration.

The Accommodations Request Form:

• Can be completed/submitted without an appointment
• Is available only to students registered with Accessibility Services
• Student must be currently receiving the requested accommodations
                                        or
• Received these accommodations during the previous Fall/Spring semester.

If you are requesting accommodations for the first time or making changes to your accommodations, please schedule an appointment with the accessibilities coordinator.

Once the form is received by Accessibility Services, a confirmation will be forwarded to the sender.  Faculty Notification forms should be available no later than two weeks after the request.  No appointment is needed. Hours for pickup – 8:30 a.m.-4 p.m. (Monday-Friday) except when the campus is closed.

Please note:  Faculty Notification forms may be delayed in completion if clarification is needed.  An appointment may be needed in some instances.
